For Licensed Clinicians, CSAT Therapists/Candidates and APSATS Certified Specialists and Coaches Only

I agree that I am either: a licensed clinician working within the scope of my professional license(s) and parameters of my governing state board(s); a certified sex addiction therapist or candidate (CSAT); or an APSATS certified partner specialist or coach.  If I am not a certified CSAT clinician, or a licensed clinician experienced with sex addiction treatment, I certify that I am working under the supervision of a certified CSAT supervisor or a licensed clinician experienced in the treatment of sexual addiction, partner trauma and disclosure facilitation.

Licensing & Ethical Guidelines

I understand that state law and ethical guidelines differ from state to state and country to country. I agree that I will check with my licensing board, membership organization or attorney and agree to make any changes needed to the documents contained in the Relational Recovery Disclosure Prep Manual in order to comply with my state board legal and ethical requirements. I also agree that I am responsible for any resulting legal or ethical issues, lawsuits with clients or others. I agree to assume all risk involved in using or not using these materials.

Appropriate Use

The Relational Recovery Disclosure Prep Manual and all information, materials, and exercises are designed for higher functioning clients who are not challenged with acute mental illness or severe personality disorders. Professional judgment, clinical assessment, and best practices should be used to screen clients.

I understand that by licensing and/or downloading the Disclosure Prep Manual or any part thereof, I may not:

  • Sell, share, lend, copy, duplicate, email, or license the Relational Recovery Disclosure Prep Manual or any part thereof to others including clinicians, professionals or clients. This license is non-transferable;
  • Remove the copyright notice on the Relational Recovery Disclosure Prep Manual or any part thereof

This license agreement does not convey or transfer any ownership or proprietary interest in any intellectual property rights. I agree not to claim copyright in, or exercise ownership of the Disclosure Prep Manual or any part thereof.

Limitation of Liability

In no event will Michelle Mays, Relational Recovery, Inc. and/or Relational Recovery Press, LLC be liable for any consequential, indirect, incidental, or special damages resulting from any claim (including any cause of action sounding in contract, tort, negligence, strict liability or products liability) arising out of the use by you of the Disclosure Prep Manual or this license agreement including, but not limited to, any lost profits or lost savings, damages resulting from business interruption,  even if Michelle Mays, Relational Recovery, Inc. and/or Relational Recovery Press, LLC have been advised of the possibility of such damages. The maximum liability of Michelle Mays, Relational Recovery, Inc. and/or Relational Recovery Press, LLC to you for any cause whatsoever will be no greater than the price paid to Michelle Mays, Relational Recovery, Inc. and/or Relational Recovery Press, LLC under this license agreement for the Disclosure Prep Manual or any part thereof. The foregoing limitations and exclusions apply to the extent permitted by applicable law.

Indemnity

I agree to indemnify Michelle Mays, Relational Recovery Inc., and Relational Recovery Press LLC from any and all judgments, liability, damages, penalties, losses and expense (including reasonable attorneys’ fees) which may be suffered or assumed by or obtained against Michelle Mays, Relational Recovery, Inc, and Relational Recovery Press LLC, their successors, licensees, and assigns that may occur as a result of my license, download, or use of the Disclosure Prep Manual or any part thereof within my practice, agency or organization.
